What health screenings do I need at 40?

The American College of Obstetricians and Gynecologists (ACOG) recommends a gynecological exam every year, and that includes a pelvic exam. A Pap smear is different from a gyno exam. Most doctors recommend a Pap smear every 3 years unless you have multiple sexual partners or a weakened immune system.

Also, make sure you get breast health screenings. While you should continue to perform monthly breast self-exams, you should also schedule annual mammograms starting in your 40s, as some cancers can’t be detected by self-exams.

Most women will begin to experience perimenopause in their 40s, which is the stage just before menopause. Symptoms of perimenopause can include:

  • Skipped or abnormal periods
  • Heavy or lighter than normal periods

Birth control in your 40s

While it’s common knowledge to most women that fertility begins to decline after the age of 35, the ability to become pregnant is determined by menopause, and that varies widely from woman to woman.

Many women are under the misconception that they can’t get pregnant in their 40s, and because of this, they opt not to use contraception.

But the reality is that unless you’re over 50 and have gone without a period for at least a year, you should still use birth control if you don’t want to get pregnant.

Birth control methods have improved for women in their 40s. While the birth control pill used to be considered unsafe for this age group, advancements have made it much safer and more effective for this age group.

Tubal removal is a popular choice of birth control at this stage in life, as are IUDs, vaginal rings, and implants.

General health recommendations for women in their 40s

Your 40s are a critical time to focus on preventive care and establish healthy routines that support long-term wellness. Here’s what healthcare providers recommend for women in this stage of life:

  • Schedule an annual physical. A yearly checkup with your primary care doctor helps track changes in your health and detect early signs of potential issues.
  • Support bone health. Bone density naturally decreases with age, so make sure you’re getting enough calcium and vitamin D through diet or supplements. Weight-bearing exercise can also help maintain bone strength and reduce your risk of osteoporosis.
  • Stay active and eat well. If you haven’t already, now’s the time to adopt a balanced diet and consistent exercise routine to support heart health, weight management, and energy levels.
  • Monitor for diabetes. A diabetes screening is recommended every 3 years starting at age 45—or sooner and more frequently if you’re overweight or at higher risk.
  • Keep up with dental visits. Schedule at least one dental exam and cleaning each year (twice a year is even better) to maintain good oral and overall health.
  • Check your blood pressure. Have it checked at least every 2 years, or more often if your readings are above 120/80.
  • Get your cholesterol tested. Have your cholesterol levels checked every 5 years—or sooner if you have heart disease risk factors.
  • Protect your skin. Do a self-skin exam each month and visit a dermatologist yearly for a professional screening. Always use sunscreen to reduce your risk of skin cancer and premature aging.

Immunizations after 40

The following immunizations are recommended for most women in their 40s:

  • Tetanus vaccine every 10 years
  • Pneumococcal vaccine (1 to 2 doses) if you smoke or have certain chronic conditions
  • Hepatitis A & B vaccine if you travel or live in an area where these infections are common, if you use illegal injection drugs, or if you have multiple sex partners

Many physicians also recommend getting a flu shot each year.

Mental health in your 40s

Many women in their 40s experience changes in mood, motivation, and emotional well-being. This can stem from a combination of factors—hormonal fluctuations, perimenopause, shifting family dynamics, career pressures, or caring for both children and aging parents.
